Behavior change

Q&A

    What is an appropriate referral?

Referrals for services from the NWSE Behavior Consultant may be deemed appropriate by member districts for two primary reasons:

    • General consultation for individuals with specific needs related to Autism or other specific disabilities for support and guidance in developing appropriate programming, goals, resources, and training.
    • Individual consultation and support for students with intensive behavioral need to assist teams in conducting Functional Behavior Assessments, assisting in writing and implementing Behavior Intervention Plans, and assisting in data collection and progress monitoring.
Where do I get referral paperwork?

Paperwork may be obtained by contacting Cassidy Caley at cassidy.caley@nwseonline.com

    What is the referral process?

STEP 1: The referring staff member must first complete the previous intervention summary page and provide specific documentation and data to provide evidence that attempts to provide intervention have been made. If all in-district supports and resources have not been completed the referral will not be accepted. All other referral packet information (parent consent form, referring staff member agreement form, general referral form) needs to be completed and returned with it. A total of four documents, with accompanying documentation, should be emailed to cassidy.caley@nwseonline.com

STEP 2: Once a complete referral is received, individual circumstances will be assessed by the NWSE Behavior Consultant to determine if services are deemed appropriate at the current time.

STEP 3: Initial consultation meeting will be set to collect information related the target behavior of concern.

STEP 4: The NWSE Behavior Consultant will recommend next steps for the team to consider.

How do I submit referral paperwork?

Submit referral requests to Cassidy Caley in the NWSE Office, at cassidy.caley@nwseonline.com. All referral paperwork, including supporting documentation, must be submitted in their entirety, before a referral will be accepted by the Behavior Consultant. Please review the referral paperwork carefully; it includes several checklists regarding what should be included/completed in a referral to ensure it is accepted.
